Mamiya-Sekor SF C 150mm 1:4 Soft Focus Lens for Mamiya RB67 with Three Soft Focus Discs. It comes with the rear cap but no front cap.
A step down 67mm ring was glued in place. That's how the lens was sold to me by a professional wedding photographer. The step down ring does not interfere with the picture taken.
I think the condition is fungus-free, but I've uploaded photos of the lens I shot against a light source.
